MEDMARX® Data Reports
Each year USP analyzes medication error records submitted by facilities participating in its national medication error and adverse drug reactions reporting program, MEDMARX®. The data is compiled, summarized and presented in an in-depth annual report. Information in the report includes types of medication errors, causes, contributing factors, products involved, and actions taken.
MEDMARX® Data Report
A Report on the Relationship of Drug Names and Medication Errors in Response to the Institute of Medicine's Call to Action (2003–2006 Findings and Trends 2002-2006)
The 2006 MEDMARX Data Report focuses on medication errors involving drug nomenclature as reported to either MEDMARX or the USP–ISMP Medication Errors Reporting Program. Analysis of more than 26,000 records with look–alike and/or sound–alike drug names indicates that the risk of a medication error increases with similar drug names. The Report also includes a drug name pronunciation guide from the USP Dictionary of USAN and International Drug Names. Historical information about medication errors from the MEDMARX database are contained in the technical appendices.
MEDMARX® Data Report
A Chartbook of Medication Error Findings from Perioperative Settings From 1998–2005
The 2005 MEDMARX Data Report provides a comprehensive analysis of more than 11,000 perioperative medication error records submitted to MEDMARX over a 7–year period. Errors were segregated by clinical location (outpatient surgery, preoperative holding area, operating room, and postanesthesia care unit) and by patient population (pediatric, adult, and geriatric). More than 45 recommendations to avoid future errors are described. Historical information about medication errors from the MEDMARX database are contained in the technical appendices.
MEDMARX® Data Report
A Chartbook of 2000–2004 Findings from Intensive Care Units and Radiological Services
The 2004 MEDMARX Data Report provides a thorough analysis of records submitted to MEDMARX specifically capturing information on Intensive Care Units, including coronary, general, medical, and surgical ICU's; and Radiological Services focusing on cardiac catheterization labs, nuclear medicine, and radiology departments. More than 40,000 records were reviewed to establish the significance of medication errors in these areas of service.
MEDMARX 5th Anniversary Data Report
A Chartbook of 2003 Findings and Trends 1999–2003
The MEDMARX 5th Anniversary Data Report provides a comprehensive analysis of 235,159 records submitted to MEDMARX in 2003 by 570 participating facilities, and also provides 5-year trend analyses based on the nearly 600,000 records submitted from 1999 to 2003. For ease of study, the data, findings, and implications are presented in a chartbook format and are accompanied by technical appendices.
The MEDMARX 5th Anniversary Data Report includes three technology-focused special topics:

- Computerized Prescriber Order Entry—analysis performed in collaboration with the Agency for Healthcare Research and Quality (AHRQ)

MEDMARX 2002 Annual Data Report
The Quest for Quality
The MEDMARX 2002 annual data report analyzes 192,477 medication errors voluntarily reported by nearly 500 participating hospitals and health systems nationwide. It provides core findings as well as detailed information about the products, populations, and priorities involved in medication errors. The 2002 report includes a synopsis of medication errors in geriatric patients.
MEDMARX 2001 Annual Data Report
A Human Factors Approach to Understanding Medication Errors
The MEDMARX 2001 annual data report analyzes more than 105,000 medication errors voluntarily reported by 368 participating hospitals and health systems nationwide. The report presents key findings and helps to prioritize actions for increased prevention of both potential and actual medication errors. The 2001 report includes a synopsis of medication errors in pediatric patients.
