ISMP List of Error-Prone Abbreviations, Symbols, and Dose Designations

It's been more than 2 years since the Institute for Safe Medication Practices (ISMP) published a list of abbreviations, symbols, and dose designations that have contributed to medication errors. With the 2004 JCAHO National Patient Safety Goals calling for organizational compliance with a list of prohibited "dangerous" abbreviations, acronyms and symbols, the ISMP has compiled an updated list, ISMP List of Error-Prone Abbreviations, Symbols, and Dose Designations  (47KB).
