MEDMARX®

MEDMARX® is a national, Internet-accessible database that hospitals and health care systems use to track and trend adverse drug reactions and medication errors. Hospitals and health care systems participate in MEDMARX voluntarily and subscribe to it on an annual basis. MEDMARX is a quality improvement tool, which facilitates productive and efficient documentation, reporting, analysis, tracking, trending, and prevention of adverse drug events.

In a decision to focus full attention and resources on its core standards setting activities, USP transferred the MEDMARX reporting program to Quantros in December of 2008. All queries about MEDMARX should be addressed to Quantros (www.quantros.com). Printed copies of past MEDMARX reports are still available for purchase in the USP store. In the interest of public health and to assist practitioners and patients, USP will soon post its five years of MEDMARX data and eight annual reports on www.usp.org in electronic format for free, ensuring full access to this clinically important information.
